Methods of Donation

- Go to any MLhuillier Kwarta Padala outlet
- Fill out the Customer’s Information Sheet and provide
the following info:
a.Amount
b.Sender’s Full name
c.Complete Address (IMPORTANT)
d.Purpose of Transactions (example: pledge)
e.Receiver’s Name (example: CBN Asia)
f.Additional Message if needed
g.Signature
- Submit the completed form to the counter form
together with your donation.
- Obtain your copy of the form as proof of your donation.
Mail
- You may send your donation in the form check or postal money order (payable to CBN Asia, Inc. Please do not enclosed cash) in the enclosed Business Reply envelope and mail to:
